Passport is the first thing you need to get. If you had a passport for a while, make sure it has couple of pages blank and at least six months validity.
Then decide which country you want to visit, all the interesting ones need visa.

You need to apply for Visa at least a month in advance and you need to book flight ticket and hotel before applying for the Visa. This too depends on the country per basis, so search online or talk to a travel agent

There are few countries where you don’t need Visa, you will get visa on arrival at the airport.
To get the visa you need to give few documents and give your original passport. You also need to give some photos, some countries also require you to visit their embassy for an interview.
It’s best to do it with travel agent atleast for the first-time
There is a visa fee depending on the country you visit, Visa is just a A4 paper giving you the details like duration, date etc with seal on it.
When you go to the airport once you take the ticket, there is the immigration counter where they will check your visa and put a seal in your passport
When you land in the other country, you will have to fill out a form on how long you will stay there etc. Your passport will be sealed with the date. You will get a slip which you need to keep safe and return it when coming back, that’s pretty much it
Carry your passport with you, don’t keep it in your room. Keep a printed copy in your baggage so that even if you lose your passport, you have a printout.
To spend money get a forex card. Which is just a debit card that can be used overseas with no extra charges. You can get this from your bank. You can recharge it through your net banking. Cancel the card once you are back from your trip. If its a short trip, credit card should be fine but usually there are few extra charges.
Keep your luggage small, have some local currency in hand in case cards are not accepted
You can use your mobile in other countries with your sim. Just make sure international roaming is activated. Recharge with an international package too. Airtel has a 10 day package for ₹1499 which gives you couple of hours talk time and 3 GB of data. This should be more than enough if you just want to use your phone for directions and Uber etc. You mostly get free wifi in hotels and restaurants etc.
You can also get new cards in the country you are traveling, but the above method is much more hassle free
Don’t forget to restart the phone once your are in other country. Some times the cellular network doesn’t automatically switch.
